To ensure that MACRA is implemented in a manner that best serves Medicare patients and the physicians
that care for our nation’s seniors, we urge Centers for Medicare & Medicaid Services (CMS) to interact
and listen to physician advocacy groups more closely; slow down the timeline for implementing a quality
scorecard; and ensure that more feasible and valid programs can mature through accredited processes
and organizations. The Neurosurgical Quality Council subcommittee of the American Association of
Neurological Surgeons and Congress of Neurological Surgeons is a case in point, whereby that specialty
has implemented a clinical data registry that addresses all the MIPS elements in a very scientiï¬c and
granular manner. They have learned a great deal in how to deï¬ne quality, create benchmarks with
appropriate risk stratiï¬cation and embrace the quality improvement tools that all neurosurgeons will need
to provide best care in real time.
We urge Congress to provide continued oversight of MACRA’s quality payment program to preclude the
CMS from implementing MIPS and APM rules that are highly burdensome, inaccurate and expensive, and
which will negatively impact physicians and patients, alike.
