        •   Incorrect blood : anticoagulant ratio
        •   Insufficient volume of specimen                                     GENERAL OPERATING POLICIES
        •   Grossly blood stained specimen
        •   Test not done / not offered / obsolete
        •   Specimen received after office hour ( test done during office hour)
        •   No FBC result and blood smear received
        •   Specimen for PT/APTT not suitable for testing (> 4 hours)
        •   No diagnosis or clinical summary of patient
        •   Test requested less than 3 months from the previous request (for certain tests)
        •   Test temporary withhold / stop (already informed)
        The ward or clinic staff will be notified if the specimen is rejected.



        RESULTS/REPORTS

        Routine test results from Chemical Pathology, Haematology and Microbiology will be
        placed in the designated pigeon hole.  These results can also be viewed and printed
        from ward view station available in designated wards/clinics.

        Result for drug abuse testing should be collected by respective wards/clinics/agencies
        from the toxicology laboratory.  HIV positive result will be despatched by hand (internal
        request) or send by post (external request).

        Histopathology, cytology and bone marrow reports should be collected by respective
        clinics/wards/departments  from  the  histopathology,  cytology  or  haematology  unit
        respectively.

        For ACC Clinical Laboratory tests, results can be obtained directly.

        All critical results will be notified to the requester immediately. (INSERT LIST)

        Enquiry of test results shall be made using the form for tracing laboratory results HTAA/
        PAT/FORM3.
