              INCIDENT REPORTING SYSTEM ( INCLUDING ROOT CAUSE

              ANALYSIS ) OR OTHER METHODS TO INVESTIGATE

              INCIDENTS ( EG: CLINICAL AUDIT, ENQUIRES ETC.)





            INTRODUCTION :



              According to the World Health Organization, (WHO) Incident Reporting
              is done to improve the safety and health system through:

            • Reporting  of incident
            • Learning from incident

            • Improvement or action taken following an incident
            • Sharing  the experience
            • Prevent recurring of incident



              MOH Incident Reporting  is divided into:
            • Mandatory where the incident must be reported immediately.

            • Voluntary where the incident  are  "near misses”, hazards and
              other incidents not listed in the mandatory lists.

              OBJECTIVES



              To report incident



              To  learn from the incident, take appropriate remedial actions,
              prevent recurring incidents , and  share  lessons from the incident.
