                                                                                     a wrong plan to achieve an aim (i.e., error in

                                                                                  • An injury to a patient caused by medical

                                                                                     management rather than the patient’s

                                                                                     underlying condition is called an adverse event

                                                                                     or a patient safety event (The Joint Commission

                                                                                     [TJC], 2016a), and most of these events are

                                                                                     preventable.
