Client Intake Interview (Template)

You have been tasked with conducting an Intake Interview for your

Foundations client. Compose an introduction that encompasses how you

would introduce yourself to your client, explain your role, and explain the

interview process.

Transition to the drafting of the Intake Interview questions using the Form

(insert location and access directions). Prepare a minimum of 25 pre-event,

25 event, and 25 post-event Intake Interview questions.

Be sure to review the Rubric as part of your assignment preparation. The

Rubric is posted in the Announcements, and is viewable in the assignment.
