My special appreciation goes to Dr. David Hassell for the chest radiographs show-
ing thoracic vascular lines. Their knowledge and experience in different aspects of
critical care have made this edition clinically relevant and practical. I also thank
other colleagues for their help in many different capacities for the last three edi-
tions. Their contribution to the process of teaching and learning is evident through-
