     coverage due to dependent’s job status change or annual enrollment.

     deleted in order to meet any state or legal requirements. The Company retains the right to amend
     or terminate benefits as it deems necessary at any time. Benefit options, eligibility and other
     details may be different for employees covered under a Collective Bargaining Agreement (CBA).
