        ASSISTING THORACENTESIS PROCEDURE



            No.                                           Procedure

             1.         Ensure physician’s order.
             2.         Greet client and introduce self.
             3.         Explain procedure to the patient.

             4.         Obtain baseline assessment and patient’s medical history.
             5.         Ensure a signed consent has been completed.
             6.         Perform hand hygiene.
             7.         Prepare trolley with aseptic technique.

             8.         Provide privacy.

                        Assist in patient’s positioning:
             9.        •  sitting up / upright position with arms on the bedside table or
                       •  lying / lateral position on the unaffected side
            10.         Expose the insertion area.
            11.         Assist doctor throughout the procedure:

                        •  maintain patient’s positioning
                        •  instruct patient to inhale deeply during the insertion
                        •  assessment of vital signs

                        •  provide emotional support
                        •  manage supplies
                        •  maintain sterile field and technique


                        Upon completion of procedure:
            12.         •  ensure occlusive sterile dressing is applied to the Thoracentesis site

                        •  position patient in comfortable position on unaffected site

            13.         Record the amount of drainage and its characteristics.

            14.         Dispose and clean the equipment according to local protocol.


            15.         Label and send out specimens as ordered.

            16.         Remove gloves and wash hands.


                        Perform assessment:
            17.         - vital signs
                        - assess patient for complications

            18.         Document and report
