Office hour : Receive all specimens for biochemical, haematology,
serology, parasitology and microbiology. Specimens for
histopathology, cytology and molecular test must be sent
directly to the respective laboratory.
After office hour : Receive all specimens except blood transfusion samples. GENERAL OPERATING POLICIES
Blood Transfusion Counter
Specimens relevant to transfusion medicine testing must be sent directly to
the Transfusion Laboratory at any time (24 hours).
PRE-ANALYTICAL REQUIREMENTS
INTRODUCTION
Pre-analytical is any procedure that takes place before an analysis. Pre-analytical
errors are estimated to constitute 50-70 % of errors. It is important to understand and
apply correct pre-analytical procedures because it may affect patient’s result.
Note: Correct patient identity is mandatory.
REQUEST FORM
A standard laboratory request form i.e PER-PAT 301 is used for all categories of tests
except otherwise stated in Page 117-162.
The following information must be provided in every request form :
i. Patient’s detail : Full name, full identity card (IC) number,
registration number (R/N), sex and
age.
ii. Source : Ward, clinic and name of hospital.
iii. Patient’s clinical summary : Relevant clinical summary including provisional
diagnosis and treatment. Abbreviations are dis-
couraged.
iv. Test details : Request must specify the test required.
v. Specimen : Date and time of specimen collection. Type of
specimen and anatomic site (if relevant).
vi. Requester detail : Doctor’s name, signature and official
stamp.
