What is Circle of Care
• Community-based non-profit agency providing
Home Health Care and Community Support
Services since 1974. • In 2013/14, 750 staff and
390 volunteers supported over 8,600 clients.
• Staff include Regulated Health Professionals;
Personal Support Workers.
• We offer a full range of services to support
individual independence so clients can live
healthier, happier and longer lives at home.

Programs and Services

• Personal Support, Homemaking and Respite

Care, Kosher Meals on Wheels, Community
Transportation, Falls Prevention and Exercise
Classes, Social Work, Day Centre for Seniors
. with Dementia, Physiotherapy and Dietetics,
Access and Navigation, Assisted Living,
Visiting Hospice, Family Caregiver Programs
(eg. Support groups and educational sessions)
• Community Nursing (available in 2015)

Location
• Services provided to clients in their homes across the Greater Toronto Area including: Toronto,
Etobicoke, North York, Scarborough, and southern York Region.
• 11 offices and program locations in Toronto, North York and York Region
