Use Legacy Veterinary Hospital Letterhead
DATE: Feb 19 2015
Dear (Client Last Name)
According to our records you had requested boarding arrangements from Feb 4 – 7 2015,
updated vaccinations, and a wellness exam for your tabby cat “Mitch”.
th
We have telephoned repeatedly in the weeks following February 9 to inquire about pickup
status, but have received no response to any of the listed phone numbers or email address you
have provided.
If you are unable to do pick up “Mitch” due to illness or a change in circumstances, and cannot
arrange for a friend to take care of your pet, we will happily bring him to you, or even arrange
for a Rescue Group to get him adopted to a loving home.
If you fail to contact us by February 27 2015, we will have to assume that you have abandoned
your pet and we will proceed to contact the Frisco Animal Control to pick up “Mitch”. If this
